Community Stabilization (Crisis)

Community Stabilization is a transition service that supports an individual who recently experienced a crisis. The individual has been deescalated from a crisis in the past 72 hours and/or is stepping down from an inpatient or residential treatment program. The individual requires support working through barriers to stability. Support available through Community Stabilization includes establishing and following a safety plan, expanding coping skills to reduce the likelihood of repeated mental health crisis, and engagement with aftercare providers for ongoing support. This service is typically provided for 3-7 days in the individual’s home or community.

Mental Health Skill-Building (Life Skills)

Mental Health Skill-Building is provided to adults (ages 18+) who require support to maintain stability in the community due to a mental health condition. Individuals who would benefit from this service have a serious mental illness and can live independently with skills training to manage symptoms and activities of daily living. Training provided will include building coping skills for symptom management, engaging with outpatient providers, managing activities of daily living and enhancing interpersonal skills. This service is typically provided for 3-10 hours per week and for 3-12 months.

Addiction Intensive Outpatient Program (Addiction)

The Addiction IOP program is an intensive group-based program for adults (18+) who are in recovery from addiction. Individuals in early recovery or who are at risk for relapse are appropriate for IOP. The service typically lasts 90 days with services provided 3 times per week in order to support the individual in their recovery. Services are provided by a licensed clinician and/or Certified Substance Abuse Counselor in a group format so that group members may support one another in recovery.

Intensive In-Home Counseling (Counseling)

Intensive In-Home Counseling is available to youth (ages 5-21) who are at risk for out of home placement. To support the individual remaining in the home, the Intensive In-Home Counselor works with the youth and family for 3-10 hours per week to improve communication and relationships. Intensive In-Home Counseling is appropriate for youth who are justice involved and/or have not been able to reduce at risk behaviors through less intensive services.
